Grind down concrete patches in 15" Parshall flume sides so that they will not protrude. 2. Cut former rectangular sed, tank roof column holddown bolts 1" below surface and grout smooth. 3. Tighten tie rod at SW corner of new headworks building extension. 4. Remove former tie rod attaching angle bracket from existing free standing 3" pipe column in center of new headworks building extension and remove old tie rod from NW corner of screening room. 5. Cover holes in existing AC siding around removed door at west side of existing headworks building and at lower left side of double door D/4. 6. Caulk along inside of top of corrugated AC siding at headworks ex- tension and around all new door frames with rubber mastic. 7. Door D/19, at west side of headworks, is missing a holder; Door D/4, at south side of headworks, is missing a drip cap and a holder and roller door D/22 is missing a bar type locking device on outside face for padlock. 8. Paint siding and cap over removed wall and roof exhaust fan openings in headworks. 9. Paint AC siding over removed doorway at west side of existing head- works building and paint top outside face of stem wall below new double door D/4. 10. Paint over welds at base of hand rail around manually cleaned bar screen. 11. Paint No. 2 bar screen motor. 12. Touch up over brown staining of grey paint around inspection doorway of No. 2 bar screen. 13. Touch up galvanizing over rusted welds on belt conveyor side rails. 14. Paint conduit supports and channel iron supports around door D/4, odor scrubber intake and across removed doorway opening. O" 15. Grout pack under and bolt 2nd screening belt conveyor to floor (use 1/2" thick grout pack min.). 16. Replace missing bolts in removable inspection cover on east side of 2nd bar screen. r+ bacK211PdG~ 36 Page Two HEADWORKS BUILDING (Cont'd.) 17. Install suitably calibrated sonic sensor over No. 1 Parshal flume. r 18. Install drive shaft cover over basement vent fan. 19. Install new 31-0" x 2'-0" adjustable and lockable louver over odor scrubber intake. 20. Trim back large protrusion on unistrut conduit floor support brackets for basement vent fan (tripping hazard). 21. Coat bare spots on pvc coated conduit at couplings where pvc sleeves were too short with two coats of epoxy paint or vinyl cap, also coat boxes and fittings on pvc coated system with two coats epoxy paint. GRIT BUILDING 1. Replace removed concrete walkway between grit building and rectangular sed. tank. 2. Bridge grating over overflow weir needs to be tied down with re- movable clips. 3. Paint new inlet louver support channel iron. 4. Paint bottom supports of bridge over overflow weir both inside and out with medium consistency coal tar. CARBON ODOR SCRUBBER 1. Install weatherproof collar around fiberglass intake duct on outside of grit building south wall. 2. Remove resin residue from concrete stem wall under fiberglass intake duct on outside of grit building south wall. 3. Paint replacement odor scrubber motor (grit building scrubber). 4. Install shaft guards over both blower drive shafts. 5. Fill both odor scrubbers with more activated carbon to achieve a three foot deep filter bed. 6. Fix flange gasket leaks on both blower discharges. 7. Install missing pipe strap oil headworks odor scrubber intake duct over pipe support as shown on drawing 49 elevation C. SITE WORK (Including 16 inch plant bypass line and electrical under- ground feeders and cables) 1. Clean up area east of new north parking area by removing gravel and cable and filling in hole against existing maintenance building north wall. ,atitraGE437 t PagQ Three SITE WORK (Cont!d.) 2. Clean up debris north of north parking area, west and east of south parking area and removed A/C pavement and overexcavated material stockpiles from north portion of treatment plant site. 3. Remove old telephone cable and 4" duct from behind (west side) of closed vehicle storage building. 4. Remove temporary timber handrail and other construction debris and damaged chlorine double door from south portion of treatment plant site. 5. Remove contractor's and electrical subcontractor's trailers and tools as well as resident engineer's trailer from the site and disconnect and remove all temporary telephone and power wiring to them. 6. Clean construction debris from around the emergency generator building. 7. Remove gravel pile east of No. 5 aeration basin. 8. Replace 1 1/2 inch rock cover north of final effluent pumping station and west of sludge holding tanks. 9. Install gravel filled infiltration trenches to east of north parking lot and to north of emergency pumping station. ~4r 10. Cut shallow drainage channel to drain standing water from over top of culvert installed east of new maintenance building. 11. Grout in discharge end of relocated 30 inch CMP into catch basin south of new chlorine building. 12. Sweep gravel off of emergency pumping station access road. 13. Install 8 inch A/C paving over 10,000 gallon fuel tank east of emergency pumping station (surface smoothness 1/4" per foot max.). 14. Extra paving to be performed at headworks, grit and chlorine buildings as per Change Order No. 17. 15. Pave over all in roadway trenches on site. 16. Pave over and fill in all bird baths on A/C paving between maintenance and vehicle storage buildings. 17. Paint parking stripes in north and south rear parking lots. 18. Construct 2 or 3 runoff diverting berms across emergency return station access road. 19. Reseed and mulch the following areas which have either been dis- ` turbed or on which the previous seeding did not hold: (a) area around and to the east of breakpoint tank and around 16 inch valve vault; ©v2111.FAGE438 Page Four SITE WORK (Cont'd.) (b) area over 1,000 gal. fuel tank south of emergency generator building; (c) area around Trout Creek pumping station; (d) area west of tertiary building; (e) strip along north side of primary clarifier and aeration basin; (f) area around emergency return station; (g) area west and east of emergency pumping station. 20. Resod bare spots and fill holes and ruts on lawn south of adminis- tration building. 21. Replace juniper bushes along south side of north plant access road. 22. Replace 1 1/2 inch rock in area west of digesters and north of final effluent pump. 23. Replace concrete walkway between grit and rectangular primary sed. tank. 24. Patch chipped portions of top of precast 16 inch valve vault between final effluent ponds. 25. Grout up holes between anchor blocks and inside of precaust vault walls in Above 16 inch valve vault. rr 26. Install 1,000 gal. fuel tank fill cap box and concrete in. 27. Patch chipped cover to electrical hand hole no. 6 on NW and SW sides. 28. Install screwed down steel cover plate over cable trench either side of 9 MCC. 29. Fully bolt down all other steel cover plates on south side of 9 MCC pad and on all electrical hand holes. 30. Paint 9 MCC dark brown. 31. Raise fire hydrant at southeast corner of closed vehicle storage building by 6 inches. 32. Install double 900 vent with screen in lid of 16 inch valve vault. 33. Reinstate or repair lawn sprinkler system in the following areas: (a) between final effluent ponds 1 & 2; (b) in lawn south of administration building; (c) along south side of north plant access road; (d) around chemical clarifier no. 2. 34. Reinstate low lights along south side of north plant access road. 35. Reinstate light pole in front of headworks building. W111nu Page Five SITE WORK (Cont'd.) 36. Supply steel cover over telephone pull box north of lime sludge storage pond west of vehicle storage buildings. 37. Replace broken concrete cover over electrical pull box northeast of existing sludge pumping station south of emergency pumping station. 38. Supply galvanized steel screw down cover over electrical hand hole no. 10. 39. Dress new signal cable in tray along wall of existing electrical pull box east of existing sludge pumping station. 40. Electrical hand holes 4, 6, 7, 8 and 10 require lid temporarily removed for inspection. 41. Install light and power transformer in 9 MCC and install covers over inside lights. 42. Missing timer tags to be installed in bar screen control panel in 9 MCC. 43. Install guard posts either side of electrical hand holes 2 & 9. 44. Cut cable and pull back into adjacent pull box; former main electrical feed cable now hanging from former terminal pole NE of Trout Creek pumping station. S EE:evn 9-23-82 MOK21-turhGE VMS CERTIFICATE PROJECT: C-06-1414-210-02 OF OCCUPANCY OR UTILIZATION DATE : September 8, 1982 Owner: South Tahoe Public Utility District, South Lake Tahoe, CA Contractor: W. R. Thomason, Inc., Martinez, CA Contract: Headstart Program Improvements - Phase I It is mutually agreed between the Owner and the Contractor to divide the responsibilities for said partially utilized portion of the project in the following manner, commencing on the date of occupancy or utilization, which shall be taken as the date of definitive substantial completion. The designated portion of the project utilized is the Headworks and Grit Building Modifications and Site Work (including 16" bypass and electrical underground feeders and cables). The Owner's responsibilities will be (a) to operate heating and ventil- ating systems in above facility, (b) assume responsibility for power requirements to above facility, (c) assume responsibility for property insurance on occupied areas, (e) maintenance of the above facility in regard to normal operations, (f) safety of District's personnel, and, (g) shall allow Contractor reasonable access to the above facility to complete or correct items on tentative correction list attached to Substantial Completion Certificate. The Contractor's responsibilities will be (a) allow access for Owner's personnel, (b) allow access for public, if applicable, (c) allow operation of subservient heating, ventilating and electrical systems even if outside occupied areas, (d) obtain the necessary endorsements on their property insurance policy as set out in Paragraph 5.06 A of the General Conditions, (e) safety of Contractor's personnel, and, (f) warranties as outlined in the contract documents for the above specific items to commence on the definitive substantial completion date. None of the above releases the Contractor of any of his obligations to perform the work in accordance with the contract documents. Unless otherwise agreed to in writing, and so inform the Engineer prior to the issuing of the definitive Certificate of Substantial Completion, the above division of responsibilities on the designated partially utilized portions of the project will be binding on the owner and the Contractor until final payment. Ettlich Culp/Wesner/Culp p DATE : INK2111?aGE 441 CERTIFICATE PROJECT C-06-1414-210-02 SUBSTANTIAL COPIPLETION DATE September 10, 1982 Contractor Contract The work performed under this contract has been reviewed and found to be sub- stantially complete. The date of substantial completion is designated as September 8, 1982 The designated area or scope of work is the Headworks and Grit Building Modifications and Site Work (including 16" bypass and electrical underground feeders and cables). This date represents the commencement of any warranties or guarantees under the conditions of the contract that are connected with the above portion of the project only. The Date of Substantial Completion of the work is the date certified by the Engineer and approved by the Resident Engineer when construction is sufficiently complete in accordance with the contract documents to allow for the uninter- rupted start and completion of subseouent subsystems in accordance with the Construction Schedule. A list of items to be completed or corrected, prepared by the Resident Engineer and verified and amended by the Engineer, is attached. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all work in accordance with the Contract Documents.
