A RESOLUTION OF THE BOARD OF DIRECTORS OF
SOUTH TAHOE PUBLIC UTILITY DISTRICT APPROVING
NOTICE OF SUBSTANTIAL COMPLETION, HEADSTART
IMPROVEMENTS, PHASE I, FOR FINAL EFFLUENT,
TROUT CREEK AND SLUDGE RECIRCULATION PUMP-
ING STATION AND SLUDGE HOLDING, TERTIARY
AND INCINERATOR BUILDING MODIFICATIONS
RESOLVED, by the Board of Directors of the SOUTH TAHOE PUBLIC
UTILITY DISTRICT, El Dorado County, California, that:
WHEREAS, the District Engineer of said District has accepted
substantial completion of the project, for FINAL EFFLUENT, TROUT
CREEK AND SLUDGE RECIRCULATION PUMPING STANTION AND SLUDGE HOLD-
ING, TERTIARY AND INCINERATOR BUILDING MODIFICATIONS, and that
same was substantially completed on the 17th day of SEPTEMBER 1982, CGJ

COMPILED: September 17 & 22, 1982
LUTHER PASS PUMPING STATION MODIFICATIONS
1. Paint new slow closing check valve dark blue.
2. Paint new portion of packing drain pipe.
3. Paint all new small valve handles red.
4. Set all thermal and vibration detectors on new pump and motor
to suitable factory approved levels and check out that they
operate satisfactorily.
5. Dowl new pump and motor to base.
6. Touch up chip marks on new pump.
7. Caulk around telemetry antenna shielded cable through wall.
8. Operating class by Motorola on operation of telmetering equipment
and interpretation of signals at plant receiving end (constant test
signal is only transmitted at present time).
9. Install missing hold down screws in S.S. cover plates over pressure
transmitting copper tube ducts.
V`„ 10. Copper pressure transmitting tubes on pumps #1 & #2 base lacks a
number of hold down clamps.
11. Demonstrate that #4 pump will operate fully automatically without
shutting down other pumps.
12. New 5 kV starter panel requires replacement edge cover strips either
side of panel as existing ones are badly bent.
13. Label all new switches, panels and operating lights.
14. Touch up damaged front of concrete base under new 5 kV starter and
paint grey.
15. Grout new conduit into east wall.
16. Wesmar indicator unit in surge relief room is not functioning; it
constantly indicates 0 reading.
17. Paint new conduit to and from Wesmar indicator unit inside surge
relief room and outside along storage tank wall and base.
18. Condulet cover missing under Wesmar indicator unit in surge
relief room.
19. Install cover plates or caulk around conduit through surge relief
room walls.
20. Tighten up #4 pump automatic discharge valve inlet flange bolts
(flange leaks).
21. Finish installing drain tubes from automatic valve.
22. Install valves on suction and discharge pressure transmitting
copper tubes at pump end.
23. Connect wires D1 and D2 to permanent positions in control panel.

FINAL EFFLUENT PUMPING STATION MODIFICATIONS
Existing Control Room
1. Paint new conduit to breakglass switch and to and from final effluent
pump control panel.
2. Touch up scratch marks on final effluent control panel.
3. Remove all unused former resistor vent ducting panels from ceiling
and cover end of vent duct over resistor panel for #3 FE motor.
Top Deck
1. Set in place guard plates over coupling access ports on FE pumps
#1 & #2 pedestals.
2. Wrap with insulation and grout into deck top motor bearing cooling
water pipes to FE pumps #1 & 2.
3. Grout up former conduit holes in deck adjacent to motors.
4. Heat tap and wrap with insulation pump column air release vents
on both new pumps.
5. Install drain to eliminate packing water ponding at base of
motor pedestal.
6. Remove piece of former conduit unistrut support at south side of
motor base under #1 FE motor.
7. Touch up scratch marks on #2 FE motor.
8. Remove former FE motor to location approved by District.
9. Grout new conduits into deck.
Dry Well (Pump Room)
1. Grout all new conduit into ceiling and walls.
2. Paint supports under new slow closing check valvesand bottom, of flanges.
3. Explain why #2 FE pump slow closing check valve closing speed
changed.
4. Paint stop cock operating handles on bearing cooling water pipes
red.
5. Remove all old conduits on ceiling over #2 FE pump discharge pipe
and plug holes.
6. Drain pipe on west side of new final effluent pump columns is in-
adequately braced.
7. No. 2 FE pump discharge flange leaks during shutdown surge.
8. Install missing pull box cover plate in filter pump dry well.
9. Paint new conduit to match ceiling in filter pump dry well.
10. Remove old conduits and plug holes in filter pump dry well.
11. Gap in control wire conduit in filter pump dry well under new
FEP control panel to be eliminated and new conduit to be jointed
to existing throuqh the floor condulet. Also, supply condulet
cover.
12. Cut off at wall and ceiling all abandoned conduits and condulets
and plug holes in filter pumping dry well.
13. Touch up paint over concrete patch over unused bubbler pipe holes
in wall between final and filter pump dry wells.

TROUT CREEK PUMPING STATION MODIFICATIONS
1. Finish painting pipes blue and handwheels red and install labeling
and arrows.
2. Install pressure gauge in discharge manifold.
W 3. Suction and discharge piping to be supported off of floor slab
or wall and not off of pump casings.
4. Fix leaks on one suction valve and one union and tee on discharge
piping.
5. Cap former 2 inch discharge pipe 6 inches above floor.
6. Grout up hole in floor around new 4" pvc discharge pipe and
install steel protective sleeve to a level 6 inches above floor
level.
7. Pipe support under 4" check valve and valve are not anchored
properly. Supports must be such that no load is transferred
to 4" pvc pipe through floor.
8. Connect new electrical supply wire to MCC.
9. Large condulet cover is missing on outside east wall.
10. Conduit to be grouted into east wall.
SLUDGE RECIRCULATION PUMPING STATION MODIFICATIONS
1. Paint new conduit to breakglass switch.
2. Grout new conduit into south wall.
SLUDGE HOLDING BUILDING MODIFICATIONS
1. Paint althread conduit support hangers on ceiling.
2. Paint unistrut conduit support on pumps.
3. Cut old conduit off at floor level and plug hole.
4. Paint new conduit along east tank wall to motor.
5. Grout in new conduits through north wall.
6. Paint underside of large pull box on north wall.
7. Paint new conduit on outside north wall.
TERTIARY BUILDING MODIFICATIONS
1. Paint new conduit along inside south wall either side of main
south entrance door.
2. Install and paint P7 electric panel and paint conduit to and
from it.
3. Install standby power wires to light panel and breakglass
switch in panel P7.
4. Install flow signal wire from existing tertiary panel to
new electrical room.
*4W 5. Paint new air ducting to 3 MCC to match adjacent wall coloring.
6. Replace cover over new 3 MCC fan motor.
7. Paint main power conduits and brackets under catwalk and
terminal boxes at 3 MCC and on outside north wall. ~p

8. Finish off rough cut screen over 3 MCC lower right hand vent opening.
9. Replace 2 condulet covers missing in conduit near 3 MCC.

11. 10 inch #3 water line to be banded and labeled and support clamp
fitted off of catwalk above.
12. Grout around main conduits through north wall.
13. Pull box on outside north wall needs cover screwed on and painted.
Conduit below pull box also requires painting.
INCINERATOR BUILDING MODIFICATIONS
1. Demonstrate working of ground break panel and shunt trip.

CERTIFICATE PROJECT: C-06-1414-210-02
OF OCCUPANCY OR UTILIZATION DATE : September 17, 1982
Owner: South Tahoe Public Utility District, South Lake Tahoe, CA
Contractor: W. R. Thomason, Inc., Martinez, CA
Contract: Headstart Program Improvements - Phase I
It is mutually agreed between the Owner and the Contractor to divide the
responsibilities for said partially utilized portion of the project in
the following manner, commencing on the date of occupancy or utilization,
which shall be taken as the date of definitive substantial completion.
The designated portion of the project utilized is Luther Pass, Final Effluent,
Trout Creek. and Sludge Recirculation Pumping Station and Sludge Holding, Tertiary
and Incinerator Building Modifications.
The Owner's responsibilities will be (a) to operate heating and ventil-
ating systems in above facility, (b) assume responsibility for power
requirements to above facility, (c) assume responsibility for property
insurance on occupied areas, (e) maintenance of the above facility in
regard to normal operations, (f) safety of District's personnel, and,
(g) shall allow Contractor reasonable access to the above facility to
complete or correct items on tentative correction list attached to
Substantial Completion Certificate.
The Contractor's responsibilities will be (a) allow access for owner's
personnel, (b) allow access for public, if applicable, (c) allow
operation of subservient heating, ventilating and electrical systems
even if outside occupied areas, (d) obtain the necessary endorsements
on their property insurance policy as set out in Paragraph 5.06 A of
the General Conditions, (e) safety of Contractor's personnel, and,
(f) warranties as outlined in the contract documents for the above
specific items to commence on the definitive substantial completion
date.
None of the above releases the Contractor of any of his obligations
to perform the work in accordance with the contract documents.
Unless otherwise agreed to in writing, and so inform the Engineer
prior to the issuing of the definitive Certificate of Substantial
Completion, the above division of responsibilities on the designated
partially utilized portions of the project will be binding on the
Owner and the Contractor until final payment.

Contract Headstart Improvements - Phase I
The work performed under this contract has been reviewed and found to be sub-
stantially complete. The date of substantial completion is designated as
September 17, 1982 The designated area or scope of
work is Luther Pass, Final Effluent, Trout Creek and Sludge Recirculation
Pumping tations and Sludge Holding, Tertiary & Incinerator Building Modifications.
This date represents the commencement of any warranties or guarantees
under the conditions of the contract that are connected with the above
portion of the project only.
The Date of Substantial Completion of the work is the date certified by the
Engineer and approved by the Resident Engineer when construction is sufficiently
complete in accordance with the contract documents to allow for the uninter-
rupted start and completion of subsequent subsystems in accordance with the
Construction Schedule.
A list of items to be completed or corrected, prepared by the Resident
Engineer and verified and amended by the Engineer, is attached. The failure
to include any items on such list does not alter the responsibility of the
Contractor to complete all work in accordance with the Contract Documents.
