A RESOLUTION OF THE BOARD OF DIRECTORS OF THE
SOUTH TAHOE PUBLIC UTILITY DISTRICT APPROVING
NOTICE OF SUBSTANTIAL COMPLETION, HEADSTART
IMPROVEMENTS, PHASE I, FOR CHLORINE BUILDINGS,
RESIDUAL ANALYZER ROOM, EMERGENCY GENERATOR
BUILDING AND ADMINISTRATION BUILDING MODIFICA-
TIONS.
RESOLVED, by the Board of Directors of the SOUTH TAHOE PUBLIC
UTILITY DISTRICT, El Dorado County, California, that:
WHEREAS, the District Engineer of said District has accepted
substantial completion of the project, for the CHLORINE BUILDINGS,
RESIDUAL ANALYZER ROOM, EMERGENCY GENERATOR BUILDING AND ADMINISTRA-
TION BUILDING MODIFICATIONS, and that same was substantially
completed on the 16th day of SEPTEMBER 1982, by the contractor:

1. Paint soffit louvers dark brown (west & east sides).
2. Dress-up concrete retaining wall over spill onto existing block
wall.
3. Paint liquid chlorine line across north side of retaining wall,
heat tape and insulate.
4. Paint straps and inside of liquid chlorine unistrut brackets.
5. Hold down strap missing on liquid chlorine first bracket.
6. Paint railing on top of retaining wall between chlorine buildings.
7. Three condulet covers missing on outside west wall.
8. Grout conduit into wall on west wall and paint support brackets
under conduit.
Inside
1. Paint bottom of conduit runs along north wall.
2. Remove temporary power line and plug hole drilled through west
block wall.
3. Bolts missing from overhead hoist rail extension in eaves.
4. Paint unistrut brackets behind new conduit on east wall.
NEW CHLORINE BUILDING
Outside
1. Install rubber or other plug in 1/2" dia. hole over lock on outside
of all doors.
2. Clean paint from door hardware and clean glass.
3. Remove 2 nails from top of north and 2 from top of east stem wall
and grind chamber all the way to the right of door D/15.
4. Label push button switches to right of doors D/14 & D/15.
5. Caulk missing at top of doors D/24 & D/15.
6. Remove wood block imbedded in stem wall east of door D/14.
7. Patch chipped double door (D/14) threshold.
8. Install wall stop instead of floor stop and install hook off
~Aw wall to hold door open.
9. Color code four vent pipes under north eaves.
10. All four vent pipes require screens over ends.

11. Motorized louver does not shut fully.
12. Supply 100% replacement light bulbs for both inside and outside.
SULFUR DIOXIDE STORAGE ROOM (101)
1. Clean white residue off of block and concrete stem wall.
2. Trolley to hoist, attachment pins are too small for retainer
pin holes so that pins are being pinched in holes. Need larger
diameter pins.
3. Remove paint off of grey plastic weather strip alongside inside
ends of motorized inlet louver.
4. Remote switch outside doors D/14 & D/23 nor thermostat will
operate fan PF-2.
5. Grout in all conduit through walls and plug old unused conduit
holes in concrete block.
6. Close SO2 liquid high pressure sensor box cover.
7. Remove masking paper from around ceiling light in NW corner and
from around conduit under ceiling.
8. Tuck in ceiling insulation around door D/14 and sky lights.
9. Monorail hoist power cable requires suitable cable strain relief
clamp on hoist trolley end.
10. Apply "2 ton capacity" stencil to monorail.
11. Adjust SO2 sniffer and demonstrate it works.
12. Remove felt pen writing from block wall and replace with proper
labels over fan push button switches.
13. Caulk around door D/23 frame.
14. Repair hole in crane trolley fiberglass cover plate.
15. Pulley guard on hook has screw missing,
16. Insultation tape is unraveling from around top of crane
operating cable.
17. Last four pair of cylinder trunions are to be set on one inch
min. grout same as remaining ones.
CHEMICAL FEED ROOM (102)
1. Unit heater mounting is loose and heater makes a noise when operating.
2. All injector piping is to be horizontally braced. Existing brackets
offer only vertical support and do not prevent rattling of whole
piping system when injectors are in operation. The danger exists
that this rattling of the piping will eventually loosen some joints.
3. Remove paint off of grey plastic weather strip alongside inside ends
of motorized inlet louver.
V' 4. Grout up holes in concrete block behind fan starter button on
west wall and light switch on east wall.
5. Two ceiling light covers are missing.
6. Sand paint runs on door D/23.

7. Paint bottom of conduit unistrut support west of evaporators.
8, Repair water damage to ceiling insultation due to hose pipe
blow off.
,%W 9. Clean white residue off of stem and block walls (also due to
hose pipe blow off).
10. Paint althread unistrut hangers two coats of epoxy enamel in
the same color as the already painted unistrut supports.
11. Remove all masking tape from conduit and across name plates.
12. Dress off overpour at floor of all equipment bases.
13. Install new heater element in 2nd chlorine evaporator.
14. Wire up both chlorine and sulphur dioxide P.R.V.'s so that they
will close on alarms listed in drawings and notify District
immediately when this is completed so that they can make some
further adjustments to alarm panel governing the closure of
the P.R.V.'s.
15. Finish painting all conduits along north side of room over elec-
trical equipment room.
16. Replace cover over open electrical box and cap open end of conduit
in NE top corner of room.
17. Remove yellow chalk marks from top of north wall.
18. Clean dust from inside of chlorinator panels.
19. Fix leak at sulphur dioxide injector feed pressure gage plugged tee.
i%W 20. Sulphur dioxide injector support is to be bolted to floor and
packed with 1/2" grout underneath.
21. Install pressure gauges on each water supply branches to all
six injectors.
22. Chlorine gas supply line to the most easterly injector needs
floor unistrut support.
23. Fiberglass grating needs additional floor supports; it deflects
excessively in places.
24. Grout up below floor level holes around PVC solution lines in
SE corner of room.
25. Paint unistrut on wall and pipe clamps supporting PVC solution
lines in SE corner.
26. Bolt all chlorination equipment to bases.
27. Connect drain in #2 chlorinator to floor drain.
28. Close all electrical control boxes in chlorinators and sulphonators.
29. Paint all valve and stop cock handles red.
30. Valve on #2 SO2 injector water inlet leaks.
31. Band label and show flow direction arrows on all piping as required
by specifications.
32. Remove duct tape from floor drain gratings.
33. Paint shims under #1 chlorine evaporator.
34. Close condulet cover behind #2 chlorine evaporator.
35. Water supply lines to back of evaporators are not adequately braced.

'ELECTRICAL EQUIPMENT ROOM (104)
1. Electrical panels P4 & P5 need breakers numbered to identify,
2. Chip excess concrete in floor from behind conduit under electrical
panels.
3. Replace one removed weather strip from entrance door D/24.
4. Remove ceiling paint from top course of block work.
5. Replace removed electrical panel cover plate.
6. Grout up hole in north wall shoring behind outlet cover plate.
7. Paint unistrut conduit support bracket.
8. Seal holes around conduit through ceiling.
9. Remove white residue off of walls and concrete splatter off of
block walls.
10. Grout up unused conduit holes in west wall.
11. Reinforce existing plywood base or set fan CF-3 on non-sagging
supports.
12. Clean debris off of plywood platform over relocated feed room
exhaust duct.
13. Change fan PF2&PF3 percentage timers to run in 30 minute cycles
and demonstrate working of system.
14. Paint four control panels.
15. Remove temporary power line to trailer and replace L7 panel cover.
16. Wire up residual analyzer and pH probe controls and flow recorder
wiring and demonstrate that controls will control chlorinator and
sulphonator feed rates and caustic soda pumping rate.
17. Plug former conduit hole in lower left side of relocated annunciator
panel.
18. Wire up new alarm panel to reset button on relocated annunciator
panel or install new reset button on new panel.
19. Wire up C12 and SO2 PRV shutoff and open push buttons in new
control panel.
20. Wire "Low SO2 Gas Pressure" alarm to redundant "SOS Leak" or
11SO2 Liquid Low Pressure" frame on relocated annunciator panel.
21. Label all push buttons, starter panels and lights not already
identified.
22. New alarm panel is to be wired to existing annunciator panel's
audible alarm or a separate audible alarm installed.
RESIDUAL ANALYZER ROOM IN EXISTING RAS PUMPING STATION
1. Paint unistrut pipe supports.
2. Pipe support missing on vertical sample pipes on north walls.
3. Grout drain pipe into floor and sample pipe and conduit through
wall.
4. Band PVC pipe to identify.
5. Paint drain pipe and conduit in dry well.

1. Caulk around PF-5 exhaust louver.
2. Place 6 inches of topsoil over 1,000 gallon fuel tank.
3. Replace 12" gravel right of access door at NW corner of building.
4. Install screens over tank vents.
5. Patch chipped block work around L/12 louver on east wall.
6. Cut off and plug hole for old telephone conduit at NE corner
under eaves and at NW corner and under administration building
eaves.
7. Install trim around removable panels on north wall.
8. Caulk or install trim around door D/20 frame, both inside & outside.
9. Clean out existing drainage channel alongside north wall.
Inside
1. Weather strip door D/20 and install missing door butt.
2. Rub down paint runs on door D/20 and frame.
3. Bolt top of new concrete masonry block to existing timber lintel.
4. Label lighting panel L10 and number to identify breakers inside.
5. Chip excess concrete in floor from behind conduit under electrical
panels.
6. Paint althread rod hangers same color as unistrut supports.
7. Install caps over exposed ends of conduits for future connections.
8. Replace repaired day tank transfer pump impeller with new one.
9. Clean up behind and under day tank and behind 10 MCC #2 FEP panel.
10. Paint exhaust pipe and supports but leave muffler unpainted.
11. Remove unused conduit and box to day tank and plug hole in pull
box on day tank.
12. Paint unistrut supports to generator conduits.
13. Clean floor.
14. Install flashing and insulating thimble around generator and gas
heater exhausts through roof.
15. Remove paint off of grey plastic weather strip alongside inside
ends of L/9 motorized inlet louver.
16. Touch up bare spots on underside of L/9 motorized louver and
give second coat of paint.
17. Touch up damaged ceiling over generator when exhaust supports
were changed.
18. Gas heater does not respond to its thermostat.
19. Clean or replace gas heater thermostat plastic see-through cover
plate.

EMERGENCY GENERATOR BUILDING (Continued)

20. Install thermostat which controls modulating dampers in 10 MCC
exhaust duct and bolt up damper linkage.
21. Touch up rough finish on PF-5 fan housing and give housing second
coat.
22. Remove 2x4 nailed to SE corner of room.
23. Hook up generator paralleling panel.
24. Supply specified watt recording charts.
25. Touch up scratch marks on panels.
26. Resolve problem of overheating 10 MCC panels and install missing
fan in right hand end panel of #2 FEP.
27. Replace thermester to #2 FEP in 10 MCC starter panel.
28. Paint sheet metal cover plates over former vent openings in top of
both 10 MCC panels.
29. Supply and install all missing panel, light and pushbutton labels.
30. Investigate possibility of increasing size of cooling collector
manifold duct over top of both 10 MCC panels to help prevent heat
buildup in panel below.
31. Investigate possibility of venting starter panel side of 10 MCC
to prevent heat buildup in it.
32. One ceiling light is missing.
33. Remove nails from wall.
34. Circuit breaker for #2 secondary pump will not stay closed in
emergency transfer switchboard.
35. Supply 100% replacement light bulbs for both inside and outside.
ADMINISTRATION BUILDING MODIFICATIONS
1. Chip excess concrete in floor from behind conduit under electrical
pull box on outside wall to left of blower room door (SE corner of
administration building).
2. Paint conduit and hangers in blower room.
3. Reinstall voltage regulator and wire up existing standby generator
to paralleling panel and install ground wire.
4. Close hole over top of 2 MCC in blower room with matching textured
wallboard.
5. Install cover plate over new plant incoming flow totalizer.
6. Supply one years 0-15 MGD chart paper for plant incoming flow recorder.
7. Demonstrate chlorine alarm panel alarms are monitored on main plant
annunciator panel.

OF OCCUPANCY OR UTILIZATION DATE: September 16, 1982
Owner: South Tahoe Public Utility District, South Lake Tahoe, CA
Contractor: W. R. Thomason, Inc., Martinez, CA
Contract: Headstart Program Improvements - Phase I
It is mutually agreed between the Owner and the Contractor to divide the
responsibilities for said partially utilized portion of the project in
the following manner, commencing on the date of occupancy or utilization,
which shall be taken as the date of definitive substantial completion.
The designated portion of the project utilized is Chlorine Buildings,
Residual Analyzer Room, Emergency Generator Building and Administration
Building Modifications.
The Owner's responsibilities will be (a) to operate heating and ventil-
ating systems in above facility, (b) assume responsibility for power
requirements to above facility, (c) assume responsibility for property
insurance on occupied areas, (e) maintenance of the above facility in
regard to normal operations, (f) safety of District's personnel, and,
(g) shall allow Contractor reasonable access to the above facility to
complete or correct items on tentative correction list attached to
Substantial Completion Certificate.
The Contractor's responsibilities will be (a) allow access for owner's
personnel, (b) allow access for public, if applicable, (c) allow
operation of subservient heating, ventilating and electrical systems
even if outside occupied areas, (d) obtain the necessary endorsements
on their property insurance policy as set out in Paragraph 5.06 A of
the General Conditions, (e) safety of Contractor's personnel, and,
(f) warranties as outlined in the contract documents for the above
specific items to commence on the definitive substantial completion
date.
None of the above releases the Contractor of any of his obligations
to perform the work in accordance with the contract documents.
Unless otherwise agreed to in writing, and so inform the Engineer
prior to the issuing of the definitive Certificate of Substantial
Completion, the above division of responsibilities on the designated
partially utilized portions of the project will be binding on the
Owner and the Contractor until final payment.

The work performed under this contract has been reviewed and found to be sub-
stantially complete. The date of substantial completion is designated as
September 16, 1982 The designated area or scope of
work is Chlorine Buildings, Residual Analyzer Room. Emergency Gen ra or _
Building and Administration Building Modifications.
This date represents the commencement of any warranties or guarantees
under the conditions of the contract that are connected with the above
portion of the project only.
The Date of Substantial Completion of the work is the date certified by the
Engineer and approved by the Resident Engineer when construction is sufficiently
complete in accordance with the contract documents to allow for the uninter-
rupted start and completion of subsequent subsystems in accordance with the
Construction Schedule.
A list of items to be completed or corrected, prepared by the Resident
Engineer and verified and amended by the Engineer, is attached. The failure
to include any items on such list does not alter the responsibility of the
Contractor to complete all work in accordance with the Contract Documents.
