Resolution 2275 ' `i y SOUTH TAHOE PUBLIC UTII I T ~ISTRICT RESOLUTION NO. 2275 I~ COUN , C+ER A RESOLUTION OF THE BOARD OF DIRECTORS OF SOUTH TAHOE PUBLIC UTILITY DISTRICT APPROVE NOTICE OF SUBSTANTIAL COMPLETION, HEADSTART IMPROVEMENTS, PHASE I, FOR SECOND FLOOR OF MAINTENANCE BUILDING INCLUDING HIGH BAYS 104 AND 115 ON FIRST FLOOR (TENTATIVE CERTIFICATE OF SUBSTANTIAL COMPLETION RESOLVED, by the Board of Directors of the SOUTH TAHOE PUBLIC UTILITY DISTRICT, El Dorado County, California, that: WHEREAS,the District Engineer of said District has accepted substantial completion of the project, for the SECOND FLOOR OF MAINTENANCE BUILDING, INCLUDING HIGH BAYS 104 and 115 ON FIRST FLOOR, and that same was substantially completed on the 3rd day of September 1982, by the contractor: W. Plug hole in block around drain pipe under sink. 2. Some cracks in floor appear to be getting wider. 3. Apply concrete hardner on stair pan concrete. 4. Door stop on north double door into welding shop is too far over, it protects electrical panels behind. 5. Check and tighten up all gear cog key setscrews in roller door operator. 6. Install weather stripping on roller doors. 7. Skylight trim on one skylight is missing (it fell off). 8. All removable railing has too much play in its retainer cups (specified 2" PVC pipe sleeve in cups are missing). Some retainer cups are also loose in concrete curb. 9. Label and hand over all maintenance building keys. 10. Both bridge and mono rail crane, trolley to hoist, attachment pins are too small for retainer pin holes so that pins are being pinched in holes. Need larger diameter pins. 11. Stencil "2 ton capacity" label on monorail. 12. Replace double element infrared heaters with single element ones ~hw on south wall. 13. Thermostat T8 on east wall is missing. 14. Supply 100% replacement light bulbs. ROOM 115 - ELECTRICAL SHOP 1. Chip excess concrete in floor from behind conduit under electrical panels and remove wire from around sink drain pipe. 2. Check and tighten up all gear cog key setscrews in roller door operators. 3. Install weather stripping on roller doors. 4. Skylight trim needs adjustment; it is sagging out of place. 5. All removable railing has too much play in its retainer cups (specified 2" PVC pipe sleeves in cups are missing). Some retainer cups are also loose in concrete curb. 6. Label and hand over all maintenance building keys. 7. Monorail crane, trolley to hoist, attachment pins are too small for retainer pin holes so that pins are being pinched in holes. Needs larger diameter pins. 8. Monorail hoist power cable requires suitable cable strain relief clamp on hoist trolley end (existing plastic straps have a tendency to break off in time). 9. Stencil "2 ton capacity" label on monorail. 10. Replace double element infrared heaters under low east ceiling with single element ones. 11. Faucet on sink leaks (indicate location of sink isolating valve to District personnel). 12. Hot and cold water pipe runs along top of wast wall sag in the middle (requires additional wall brackets). 13. Main breaker panel needs label, its breakers numbered and has a screw missing from front cover plate. ~4r 15. Supply 100% replacement light bulbs. AREA 205 - STORAGE 1. Remove foam packing from around conduit through floor at center of north wall. 2. Repoured floor topping slab west of compressors to receive one coat of grey epoxy floor covering. 3. Epoxy coating to be brushed right up to edge of wall (no gaps). 4. Finish grinding off epoxy ridges and recoat loading area passageway with grey epoxy coating. 5. Install wood or pvc base along concrete block wall to cover up rebar slots cut in it. 6. Clean up walls, remove white powder. 7. Clean paint off of plastic seal strips on sides of motorized vent louvers. 8. Paint brackets and touch up paint on compressors. 9. Paint concrete block in niche at ends of monorail white to match drop ceiling. 10. Paint copper cooling water tubing to compressors. 11. Paint electrical panel P-3 cover plate and door. 12. Bell up required at floor drain for compressor drain piping (cannot see if any drainage is taking place at present time). 13. Compressor No. 1 leaks oil at the top (oil in cylinder?). 14. Compressor piping requires additional bracing and vibration isolator TkW on cooling water tubing. 15. Compressors to operate in lead/lag mode depending on tank pressure with manually changeable lead compressor switch (presently both compressors come on at same time). 16. Three out of the four crank case and second stage compression pressure gages on the compressors do not register. 17. Vent louver operating connecting rods are not bolted up. 18. Unit heater is operating continuously; it is not thermostatically controlled as specified. 19. Label electrical power panel P-3. 20. Supply 100% replacement light bulbs. AREA 201 - MAINTENANCE STORAGE 1. Clean debris from behind Zone 2 electric furnace. 2. Brush epoxy coating right up to edge of wall (no gaps). 3. Finish epoxy coating ground down floor between storage shelves. 4. Install wood on pvc base along concrete block wall to cover up rebar slots cut in block and along base of sheet rock wall. 5. Clean up walls. 6. Remove masking tape from window frames and bottom of shelves. 7. Provide insect screens on all windows (up and downstairs). 8. Replace drop ceiling panel over Zone 3 electric furnace. 9. Dilectric coupling on large hot water heater inlet and outlet pipe is corroding badly (washer left out). t 10. Label summer and winter setting on electric furnace vent ducting. 11. Exhaust fan thermostat is wired backwards (fan comes on and louvers open at low instead of high temperature). 12. Unit heater is operating continuously; it is not thermostatically controlled as specified. BooK2111PAGE406 AREA 201 - MAINTENANCE STORAGE (Cont'd.) 13. Label locker room and seond floor exhaust fan starter panels. 14. Locker room exhaust fan does not work in automatic mode (timer downstairs does not appear to be working). 15. Zone 2 and 3 electric furnaces are not controlled by their respective thermostats (Zone 2 thermostat (in 2nd floor training room) is still missing, Zone 3 furnace does not produce any heat). 16. Electrical power panel P-2 needs its breakers numbered. 17. Supply 100% replacement light bulbs. AREA 206 - HEAVY EQUIPMENT STORAGE 1. Epoxy coating to be brushed right up to edge of wall (no gaps). 2. Finish grinding off epoxy ridges and recoat loadipg_ar-ea_passage- way with grey epoxy coating (heavy equipment sotrage area west of passageway need not be recoated). 3. Install wood on pvc base along concrete block wall to cover up rebar slots cut in it. 4. Clean up walls, remove white powder. 5. Outside door frame requires calking at top, both inside and outside. 6. Clean paint off of plastic seal strips on sides of motorized vent louvers. 7. Paint concrete block in niche at ends of monorail white to match drop ceiling; also paint electrical pull box in niche white. 8. Remove masking paper from unit heater bracket. 9. Label summer and winter setting on electric furnace vent ducting. 10. Vent louvers operating connecting rods are not bolted up. 11. Unit heater does not work. 12. Zone 1 electric furnace is not controlled by its thermostat (it runs continuously for few days and then trips breaker and shuts down). 13. Supply 100% replacement light bulbs. EE:evn m 9-10-82 BOOK2111PASE4V 1 CERTIFICATE PROJECT: C-06-1414-210-02 OF OCCUPANCY OR UTILIZATION DATE: September 3, 1982 Owner: South Tahoe Public Utility District, South Lake Tahoe, CA Contractor: W. R. Thomason, Inc., Martinez, CA Contract: Headstart Program Improvements - Phase I It is mutually agreed between the Owner and the Contractor to divide the responsibilities for said partially utilized portion of the project in the following manner, commencing on the date of occupancy or utilization, which shall be taken as the date of definitive substantial completion. The designated portion of the project utilized is the second floor of the maintenance building including high bays 104 and 115 on first floor. The Owner's responsibilities will be (a) to operate heating and ventil- ating systems in above facility, (b) assume responsibility for power requirements to above facility, (c) assume responsibility for property insurance on occupied areas, (e) maintenance of the above facility in regard to normal operations, (f) safety of District's personnel, and, (g) shall allow Contractor reasonable access to the above facility to complete or correct items on tentative correction list attached to Substantial Completion Certificate. The Contractor's responsibilities will be (a) allow access for owner's personnel, (b) allow access for public, if applicable, (c) allow operation of subservient heating, ventilating and electrical systems even if outside occupied areas, (d) obtain the necessary endorsements on their property insurance policy as set out in Paragraph 5.06 A of the General Conditions, (e) safety of Contractor's personnel, and, (f) warranties as outlined in the contract documents for the above specific items to commence on the definitive substantial completion date. None of the above releases the Contractor of any of his obligations to perform the work in accordance with the contract documents. Unless otherwise agreed to in writing, and so inform the Engineer prior to the issuing of the definitive Certificate of Substantial Completion, the above division of responsibilities on the designated partially utilized portions of the project will be binding on the Owner and the Contractor until final payment. The date of substantial completion is designated as September 3, 1982 The designated area or scope of work is the second floor of the Maintenance Building including High Bays 104 & 115 on first floor. This date represents the commencement of any warranties or guarantees under the conditions of the contract that are connected with the above portion of the project only. The Date of Substantial Completion of the work is the date certified by the Engineer and approved by the Resident Engineer when construction is sufficiently complete in accordance with the contract documents to allow for the uninter- rupted start and completion of subsequent subsystems in accordance with the Construction Schedule. A list of items to be completed or corrected, prepared by the Resident Engineer and verified and amended by the Engineer, is attached. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all work in accordance with the Contract Documents.
