Resolution 2270 RESOLUTION NO. 2270 A RESOLUTION OF THE BOARD OF DIRECTORS OF THE SOUTH TAHOE PUBLIC UTILITY DISTRICT APPROVING NOTICE OF SUBSTANTIAL COMPLETION, HEADSTART PROGRAM, EMERGENCY RETURN STATION AND HOLDING POND MODIFICATIONS, PHASE I, W.R. THOMASON, INC. RESOLVED by the Board of Directors of the SOUTH TAHOE PUBLIC UTILITY DISTRICT, El Dorado County, California, that: WHEREAS, the District Engineer of said District has accepted substantial completion of the Project, the emergency return station and holding pond modifications, phase I, W.R. The date of substantial completion is designated as July 26, 1982 The designated area or scope of work is emergency return station and holding pond modifications- This date represents the commencement of any warranties or guarantees under the conditions of the contract that are connected with the above portion.of the project only. The Date of Substantial Completion of the work is the date certified by the Engineer and approved by the Resident Engineer when construction is sufficiently complete in accordance with the contract documents to allow for the uninter- rupted start and completion of subsequent subsystems in accordance with the Construction Schedule. A list of items to be completed or corrected, prepared by the Resident Engineer and verified and amended by the Engineer, is attached. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all work in accordance with the Contract Documents. Remove old scrap pipe and barricades from site. 2. Set meter box CI cover frame in 1=2 cement mortar. 3. Dress off concrete overpour at bottom of wall at dry well floor level. 4. Clean dry well floor and grating above. 5. Remove nails from dry well wall. 6. Apply concrete hardner to dry well floor. 7. Fix eaves stain which was discolored by white patches caused by overspraying wall waterproofing compound. 8. Remove efflorescence from inside CMU walls. 9. Install missing grating over dry well. 10. Split grating either side of valve stems. 11. Band around any large holes through grating and form notches in grating for around conduits so that grating can be removed without removing conduits. 12. Grating needs to be tied down with clips. 13. Type "H" pipe support is missing from under center of 16" inter- connecting pipe in dry well. 14. Caulk to seal gap at top of wall around steel lifting beams. 15. Caulk around all pipes through dry well wall. 16. Tag and turn all keys over to District. 17. Clean paint off of limit torque motor control push buttons. 18. Touch up paint work on 3 way plug valve. 19. Paint sump pump discharge piping. 20. Clean. paint off of plastic seal strip on sides of motorized vent louver. 21. Address Motorola telemetering station and check radio for signal transfer and put unit on line; also label arial off switch. 22. Supply O&M information on 3 way plug valve's limit torque motor drive. 23. Supply new 3 way plug valve indicator plate and check location of plug relative to indicator. Relabel valve to read opening 3-1 & 2-3. 24. Connect mechanical vent louver operating linkages between louver and motor. 25. Replace unit heater as it does not work (wrong power rating). 26. Supply and install thermostat for unit heater. 27. 12" plug valve appears to be blocked and does not open fully. 28. Outside light at rear of building does not work. 29. Outlet installed in building is rated at 15 amp, not 20 amp as specified. 30. Caulk around grounding cable wall penetration. 31. Clean up around conduit through wall holes. 32. The bell box to the exhaust fan motor is missing a cover plate. 60 MG EMERGENCY POND 1. Install gate in fence at bottom of stair and install barbed-.wire strands at side of gate to prevent climbing around gate. 2. Concrete in gate fence post bases to prevent them from wobbling. 3. Replace missing top fence rail sleave about 40 ft. north of embankment stair. 4. Anchor top rail of fence to stair gate post. 5. Remove shipping tags. 9. Set high level float switch alarm 12" below existing pond overflow level. 10.
