RESOLVED, by the BOARD OF DIRECTORS of the SOUTH TAHOE PUBLIC UTILITY DISTRICT, El Dorado County, California, that: WHEREAS, the District Engineer of said District has accepted substantial completion of the PROJECT - THE EMER- GENCY PUMPING STATION HEADSTART IMPROVEMENTS PHASE I, and that same was completed on JUNE 23, 1982, by the contractor, namely: W.R. Remove portable "Sani Hut" latrine. 2. Remove spoil pile, tree stumps and metal container near pumping station. 3. Remove 02 bottles and all other construction materials from pump station building. 4. Install 8" A/C pavement over burried fuel tank west of pumping station. 5. Stop leak through wet well/dry well wall next to future 24" wall pipe and in NE corner of dry well and any other obvious place. 6. Plug drain hole in SE corner of dry well and hole in N stem wall in engine room. 7. Grout around vent pipe from duplex pump and other pipes where they go through the engine room floor and stem wall. 8. Touch up outside lip of concrete exhaust cover pipe. 9. Remove plastic stopper embedded in dry well ceiling. 10. Dress off west dry well wall. 11. Dress off dry well floor to specified finish or cover with ~4w thick coat of epoxy paint. 12. Repair holes in floor at threshold to west entrance door. 13. Remove blocking and install removable plugs over future pump hold down bolt holes. 14. Install removable plugs over future conduit holes in engine room floor. 15. Remove nails at head of dry well stairs and dress off rough concrete finish. 16. Remove nails from outside face of west stem wall. 17. Remove wood blocking behind 16" discharge pipe along south wall of dry well. 18. Cut off and patch original pump rebar dowel west of pump base in dry well. 20. Dress off curb around stairwell. 21. Remove and patch wood block from outside face of stem wall to the east of south double access door. 22. Remove wood and nails from top of south side of wet well. 23. Cast fuel dispenser base and pads around fuel tank fill cap and 30" plug valve, valve box. 24. Clean up concrete masonry units in recess either side of portal frame supporting crane rail or cover recess with approved painted cover plate. -2- 25. Patch holes in concrete masonry units. 26. Patch mortar around concrete exhaust cover pipe. 27. Remove efflorescence above west access door on inside wall. 28. Apply second damp proof coat to exterior and interior concrete masonry units. 29. Also apply damp proof coat to inside dry well walls. 30. Remove paper and clean handrailing. 31. Install expanded metal plate guard across top of right angle gear support joists. 32. Install expanded metal guard plate over drive shaft south of right angle gear drive and across engine exhaust manifold. 33. Install 2 bar railing 12" high over top of wet well stem wall. Railing is to be supported from top of stem wall only at max. 5 ft. centers. 34. Cut off excess length of 16" discharge pipe tie rod in west wall of dry well. 35. Caulk around both 30" inlet pipes into wet well. 36. Screws in all door hinges. 37. Both south access double door stops are loose. 38. South double door binds at the top. 39. West single door binds at the bottom threshold. ~Jw 40. South double door cover strip is loose and has some screws missing. 41. West single door is missing a doorstop and drip cap. 42. Clean up and touch up scuffed paint on pump casing. 43. Paint stair support beam under grating with aluminum paint. 44. Touch up scuff marks on sub-station panel in engine room. 45. Touch up brown eaves stain where it has been stained white by wall waterproofing solution. Also clean waterproofing stain from top of bronze louvers. 46. Paint soffet louver in north eaves brown. 47. Paint level sensing support bracket in SW corner of wet well. 48. Paint bottom of large movable louvers on inside face. 49. Install fuel dispenser including hose and nozzel and wire up. 50. Tag and turn all keys over to District. 51. Wire up oil temperature and supply and wire up oil level switch to right angle drive. 52. Complete all wiring up of engine control wires, taping up extra wires and closing engine junction box and control panel. 53. Install permanent seal water piping to double mechanical seal on pump and from double mechanical seal to floor drain. Backfill open seal water trench outside pumping station and reestablish drainage swale in area north of pumping station. 54. Supply and install vent cap on right angle gear drive. M . • -3- 55. Both cooling water pre--heaters on engine do not appear to work. 56. Move or permanently insulate electrical conduits and junction box adjacent to engine muffler. 57. Muffler to be sand blasted to bare metal and repainted with hi heat aluminum paint (existing prime coat was not suitable),, or completely insulated muffler to protect nearby electrical con- duits mentioned in Item 56 above. 58. Lightly oil drive shaft and universal joints. 59. Install hole through grout under main pump pedestal frame in order to drain ponding under pump pedestal. 60. 16" Mag-meter integrator seems to register in 100 gallons instead of indicated 1000 gallons. 61. Engine hours run meter does not appear to work. 62. Mag meters to be fitted with on/off switch to shut them down at no flow as they are reported to register erratically at no flow unless -shat off. 63. Supply portable ultrasonic mag meter electrode cleaner. 64. Replace broken glass cover plate over 10,000 gal. fuel gauge. 65. Reposition low level cut out float switch in NW corner of wel well. 66. Weld up last link in all adjusted operating chains on crane and valves. ~iw 67. Repair unit heater motors and install missing operating thermostats. 68. Free up operation of all louvers by scraping paint off of plastic seal strip. 69. Bolt down access hatch on duplex sump pumps cover plate. 70. Set duplex sump pumps float switch and cut off excess length of float switch rod so that only one pump will come on at a time; second pump to come on if sump level keeps rising. 71. Tighten exhaust fan, fan belt, and repair fan thermostat. 72. Turn 16" discharge plug valve operating chain drive 1800 away from wall. 73. Provide manual air bleed on main pump"s discharge pipe. 74. Remove slow closing check valve limit switch and hand to District. 75. Remove all shipping tags and labels. 76. Large manually operated louver needs operating handle. 77. Install reducing washers at end of flexible conduit to back of NW unit heater. 78. Install alternating lead pump switch on starters to duplex sump pumps or install hours run meters on both sump pumps. 79. Clean dust and debris off top of panels and light shields and replace all broken lights and light shields. 80. Check out and operate 1000 gal. fuel tank auto shutoff switch and fuel transfer pump. -4- 81. Install suitable labels on all panels and under all control lights and switches. X82. Volt meter on substation does not work. 83. Junction box cover is missing on wet well level sensing transmission system. 84. Light over south access double door does not work. 85. Install removable caps over unused conduit holes through engine room floor slab. 86. Conduit cover is missing in NW corner of dry well ceiling. 87. Cover large and small junction boxes in at ceiling of dry well. 88. Remove wood blocks and temporary support wires from dry well ceiling conduit runs. 89. Install index in all electrical breaker panels. 90. Replace all 15 amp outlets with specified 20 amp outlets (2 in dry well and 2 in engine room). 91. Install missing 24 volt DC louver motor and connect all louver operating levers. 92. Supply and install time clock on lower level exhaust fan. 93. Label and turn over all spare parts and specified maintenance materials to District. 94. Supply 100 replacement light bulbs and fuses. EE:evn 7-13-82 CERTIFICATE PROJECT: C-06-1414-210-02 OF OCCUPANCY OR UTILIZATION DATE: June 23, 1982 Owner: South Tahoe Public Utility District, South Lake Tahoe, CA Contractor: W. R. Thomason, Inc., Martinez, CA Contracti Headstart Program Improvements - Phase I It is mutually agreed between the Owner and the Contractor to divide the responsibilities for said partially utilized portion of the project in the following manner, commencing on the date of occupancy or utilization, which shall be taken as the date of definitive substantial completion. The designated portion of the project utilized is the.Emergency Pumping Station. The Owner's responsibilities will be (a) to operate heating and ventil- ating systems in above facility, (b) assume responsibility for power requirements to above facility, (c) assume responsibility for property insurance on occupied areas, (e) maintenance of the above facility in regard to normal operations, (f) safety of District's personnel, and, (g) shall allow Contractor reasonable access to the above facility to complete or correct items on tentative correction list attached to Substantial Completion Certificate. The Contractor's responsibilities will be (a) allow access for owner's personnel, (b) allow access for public, if applicable, (c) allow operation of subservient heating, ventilating and electrical systems even if outside occupied areas, (d) obtain the necessary endorsements on their property insurance policy as set out in Paragraph 5.06 A of the General Conditions.,..-(e) safety of Contractor's personnel, and, (f) warranties as outlined in the contract documents for the above specific items to commence on the definitive substantial completion date. None of the above releases the Contractor of any of his obligations to perform the work in accordance with the contract documents. Unless otherwise agreed to in writing, and so inform the Engineer prior to the issuing of the definitive Certificate of Substantial Completion, the above division of responsibilities on the designated partially utilized portions of the project will be binding on the owner and the Contractor until final payment. The date of substantial completion is designated as June 23, 1982 The designated area or scope of work is the Emergency Pumping Station This date represents the commencement of any warranties or guarantees under the conditions of the contract that are connected with the above portion.of the project only. The Date of Substantial Completion of the work is the date certified by the Engineer and approved by the Resident Engineer when construction is sufficiently complete in accordance with the contract documents to allow for the uninter- rupted start and completion of subsequent subsystems in accordance with the Construction Schedule. A list of items to be completed or corrected, prepared by the Resident Engineer and verified and amended by the Engineer, is attached. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all work in accordance with the Contract Documents.
