Resolution 2259 r F RESOLUTION NO.2259 A RESOLUTION OF THE BOARD OF DIRECTORS OF THE SOUTH TAHOE PUBLIC UTILITY DISTRICT ACCEPT- ING COMPLETION AND FILING THE NOTICE OF SUBSTANTIAL COMPLE- TION FOR: Designated area or scope of work is the 15 KV Service and Switch- gear system. RESOLVED, by the BOARD OF DIRECTORS OF THE SOUTH TAHOE PUBLIC UTILITY DISTRICT, El Dorado County, California, that: WHEREAS, the DISTRICT ENGINEER OF SAID DISTRICT HAS ACCEPTED COMPLETION OF THE PROJECT AS STATED SO ABOVE, AND THAT SAME WAS SUBSTANTIALLY COMPLETED ON THE 20TH DAY OF MAY 1982, by the contractor, namely: W. The date of substantial completion is designated as May 20, 1982 The designated area or scope of work is the 15 kV Service and sw;tchgear swctem %W This date represents the commencement of any warranties or guarantees under the conditions of the contract that are connected with the above portion.of the project only. The Date of Substantial Completion of the work is the date certified by the Engineer and approved by the Resident Engineer when construction is sufficiently complete in accordance with the contract documents to allow for the uninter- rupted start and completion of subsequent subsystems in accordance with the Construction Schedule. A list of items to be completed or corrected, prepared by the Resident Engineer and verified and amended by the Engineer, is attached. The failure N to include any items on such list does not alter the responsibility of the W Contractor to complete all work in accordance with the Contract Documents. Seal conduits with flexible sealing compound in secondary leads in 15 kV Switchgear to transformers, existing trans- former pull box and into existing transformer casing. 2. Seal the base of 15 kV switchgear and new 15 kV transformer. 3. Caulk around conduit encasement concrete plug inside special 15 kV handholes 1 and 2. 4. Paint exposed conduit pull boxes around existing transformer and new 15 kV transformer with 2 coats of chocolate brown (P-1) alkyd enamel. 5. Touch up scratch marks on 15 kV switchgear. 6. Paint special 15 kV handholes 1 and 2 lids with 2 coats of aluminum paint. 7. Straighten out damaged cooling vanes on new pad mounted transformer. 8. Provide laminated phenolic nameplates on outside doors (front and back) and also on inside passageway doors and under switches, lights and gauges of 15 kV switchgear. 9. Provide typewritten circuit directory inside 15 kV switchgear doors. 10. Auxiliary power outlets inside 15 kV switchgear are to be 20 amp rated (15 amp installed), 11. Install three lightning arrestors inside pad mounted transformer. 12. Lights inside 15 kV switchgear operate at + half power, 13. Inner panel lights inside 15 kV switchgear operate at + half power. 14. Ground fault test button on receptacle circuit does not light up. 15. AC amp meter on primary side in 15 kV switchgear does not register correctly on any of the three phases (meter does not zero properly either). 16. Kilo watt hour meter on primary side in 15 kV switchgear does not register at all. 17. Watt meter on secondary supply side of 15 kV switchgear to existing transformers 1 and 2 does not work correctly ; watt meter to new transformer 3 also needs testing )at time of inspection it could not be tested as transformer 3 was not draining a load). ~a A203n PAGE356 -2- 18. Two replacement fuses required for secondary metering system r in 15 kV switchgear. 19. Suitable fuse pulled or hot stick for each type fuse in both 15 kV switchgear and transformer. 20. One operating spring charging handle. 21. Switch operating handle for each fuse switch. 22. Set of 15 kV grounding cables. 23. One lock stick for 15 kV fuse holder and tap changer in pad mounted transformer. 24. One gallon of each color and type paint with required thinner for 15 kV switchgear, 25. One quart of each color and type paint with required thinner for 15 kV transformer. 26. Supply 100% replacement light bulbs. 27. Supply five copies of installation inspection report on 15 kV switchgear and gransformer by manufacturer's field representative (see Section 01400-Clause 2.03C of specifications). err EE:evn 6-29-82~~ FA35 f, CERTIFICATE PROJECT: C-06-1414-210-02 OF OCCUPANCY or Utilization DATE: May 20, 1982 Owner: South Tahoe Public Utility District, South Lake Tahoe, CA Contractor= W. R. Thomason, Inc., Martinez, CA Contracti Headstart Program Improvements - Phase I It is mutually agreed between the Owner and the Contractor to divide the responsibilities for said partially utilized portion of the project in the following manner, commencing on the date of occupancy or utilization, which shall be taken as the date of definitive substantial completion. The designated portion of the project utilized is the 15 kV Service and Switchgear System. The Owner's responsibilities will be (a) to operate heating and ventil- ating systems in above facility, (b) assume responsibility for power requirements to above facility, (c) assume responsibility for property insurance on occupied areas, (e) maintenance of the above facility in regard to normal operations, (f) safety of District's personnel, and, (g) shall allow Contractor reasonable access to the above facility to complete or correct items on tentative correction list attached to Substantial Completion Certificate. 1% The Contractor's responsibilities will be (a) allow access for Owner's personnel, (b) allow access for public, if applicable, (c) allow operation of subservient heating, ventilating and electrical systems even if outside occupied areas, (d) obtain the necessary endorsements on their property insurance policy as set out in Paragraph 5.06 A of the General Conditions.,,.-(e) safety of Contractor's personnel, and, (f) warranties as outi-ined in the contract documents for the above specific items to commence on the definitive substantial completion date. None of the above releases the Contractor of any of his obligations to perform the work in accordance with the contract documents. Unless otherwise agreed to in writing, and so inform the Engineer prior to the issuing of the definitive Certificate of Substantial Completion, the above division of responsibilities on the designated partially utilized portions of the project will be binding on the Owner and the Contractor until final payment.
