ITEM- PROJECT NAME: 2014 Construction Inspection and Materials Testing Services
REQUESTED BOARD ACTION: Issue Task Order No. 12 to Construction Materials Engineers,
Inc. (CME) to provide construction inspection and materials testing services on an as-
needed basis for 2014.
DISCUSSION: The District's Capital Improvement Projects under construction in the 2014
season consist of the 2014 State Streets Waterline, 2014 Saddle/Keller Waterline, and the
2014 Meter Installation Projects. In addition to these projects the City will be completing
the Bijou Erosion Control project and the Harrison Avenue Streetscape projects. A
private developer will be constructing a loop of the water main on Highway 50 at the
Chateau. Due to the number and complexity of these construction projects for the
2014 construction season, we anticipate a need for additional construction inspectors.
At the District's request, CME has submitted the attached fee estimate for services
during the 2014 construction season in the amount of$56,215. This amount is based
upon use of supplemental inspection services for 10 weeks and the materials testing
expected on the projects listed above. Supplemental inspection staff and materials
testing will be utilized only on an as-needed basis.
Staff requests that the Board approve Task Order No. 12 for CME in the amount of
$56,215.

ITEM-PROJECT NAME: Groundwater Protection Program- Drinking Source Water
Protection Area (DWSPA) Update
REQUESTED BOARD ACTION: Authorize an increase in the amount of $1,000 to Purchase
Order P26378A to Gnomon, Inc., for added GIS services, raising the contract ceiling
amount to a total of $20,750.
DISCUSSION: On May 13, 2013, the Board authorized staff to issue a task order to
Gnomon Inc., to provide professional services needed to update the District's DWSPA
datasets, map and GIS tool. During the progress of this work Gnomon, Inc. was required
to write two separate scripts for the custom mapping tool in order to be compatible
with two separate versions of ArcMap (version 10.0 and 10.2).This required additional
programming was not anticipated in the original scope of work.
As this extra work was not anticipated and was not included in the original scope of
work, staff recommends that the Board approve the requested fee increase. Approval
of the fee increase will raise the contract ceiling amount of Purchase Order P26378A to
a total amount of$20,750.

ITEMS FOR BOARD ACTION:
a. 2014 Water Meter Installation Project-Staff gave an overview of the electronic bid process
' explaining that at the time of the bid, Aspen Developers, Inc. (Aspen) was identified as the
apparent low bidder at $2,998,593. Upon review of the "good-faith-effort" �GFE) provided by
� Aspen, staff has determined that Aspen's GFE does not meet the contract requirements,
specifically the requirements to register the project at least 30 days prior to the bid date in
` order to provide outreach for Disadvantaged Business Enterprises. As such, staff finds the
Aspen bid to be non-responsive to the requirements of Section 00 74 00, Part 2.l OB(1) of the
bid documents.
The next apparent low bidder is Vinciguerra Construction. Staff has reviewed the GFE provided
by Vinciguerra and found that it does meet the contract requirements. One minor irregularity
was found, a missing phone number.Staff recommends that the Board waive the minor
irregularity and award the project to Vinciguerra in the amount of$3,172,785, as the lowest
responsive, responsible bidder.

Kevin Brazell, Vice President of Aspen Developers, gave an overview of Aspen's efforts in
; meeting the GFE and requested the Board look carefully at the bid process and GFE provided.
After further discussion, staff recommended continuing the Agenda Item to the May l sr
: Regular Board Meeting to provide Aspen the opportunity to provide proof that they are in
� compliance with the GFE requirements in the bid documents. Staff would require Aspen to
return all necessary documents to District staff by 10 a.m. on Wednesday, April 23, 2014.
; By consensus, the Board will continue the Agenda Item to a Special Board Meeting on April 23,
2014, at 2 p.m. to proceed with acquiring further documentation from Aspen to determine if
they have completed the adequate GFE effort to comply with the bid document. The second
low bidder will be notified of this continuance.
b. Truckee Marsh Sewer Facilities Protection Project- Public Hearing
� Staff gave a PowerPoint presentation on the project followed by an overview by Danielle
Hughes, Associate Hydrologist/Geologist, AECOM.
� President Schafer opened the p�blic hearing at 3:04 p.m. to receive public comments on the
Draft Initial Study/Initial Environmental Checklist and the Mitigated Negative Declaration of
` Environmental Impact for the Truckee Marsh Sewer Facilities Protection Project. No public
comments were received and the public hearing was closed at 3:04 p.m.

Staff gave a legislative update. ACWA is requesting support letters for Water Bonds AB 2686 and SB
1250. By consensus, the board approves submitting letters of supporf for Bonds AB 2686 and SB 1250.
Staff gave an update regarding the City Fire Department fire line issue discussed during the April 3,
2014 meeting.
Attorney Kvistad gave an update on the ACWA recommendation for ground water sustainability.

ITEM-PROJECT NAME: Truckee Marsh Sewer Facilities Protection Project
REQUESTED BOARD ACTION: (1) Hold a Public Hearing to receive comments on the
Initial Study (IS)/Mitigated Negative Declaration (MND) and Initial Environmental
Checklist (IEC); (2) Approve the IS/MND/IEC; Cerfify the IS/MND; (3) Certify the Notice of
Determination (NOD); and (4) Authorize the filing of the NOD with the State
Clearinghouse and EI Dorado County.
DISCUSSION: The District has prepared an IS/MND/IEC with a finding that the proposed
project will not result in any significant adverse environmental effects as a result of
implementation of the mitigation measures identified in the document. A Notice of
Availability (NOA) and Intent to Adopt a MND was filed with the State Clearinghouse on
April 1, 2014, for a 30-day public review period that expired on April 30, 2014. A Board
meeting was held on April 17, 2014, to accept comments on the IS/MND/IEC. A public
hearing was also convened on March 10, 2014, to provide neighboring property owners
an opportunity to review and comment on draft engineering plans prepared for the
project. Comments received from the neighborhood meeting were addressed by
design changes in the engineering plans, which were used for environmental
evaluation. A supplemental memorandum addressing comments received during the
30-day public review period will be prepared.
At 2:30 p.m., open the hearing to receive any additional comments on the IS/MND/IEC,
consider the comments, and the finding of no significant adverse environmental
impacts. After receiving and considering the comments, the Board may close the
hearing, certify the MND, and cerfify the NOD for the project.

The South Tahoe Public Utility District(District)has prepared an Initial Study/Mitigated Negative Declaration
� (IS/MND)in accordance with the California Environmental Quality Act(CEQA)and an Initial Environmental
� Checklist(IEC) in accordance with the Tahoe Regional Planning Agency(TRPA)Code of Ordinances,that
` identifies potentially adverse impacts related to the Upper Truckee Marsh Sewer Facilities Protection Project
(proposed project).The MND also identifies mitigation measures that would reduce or eliminate these impacts.
� The project is proposed by the District to protect its existing force and gravity sewer mains along with its
� Bellevue Pump Station facility.

Section 21081.6 of the California Public Resources Code and Sections 15091(d)and 15097 of the State CEQA
� Guidelines require public agencies"to adopt a reporting and monitoring program for changes to the project which
.; it has adopted or made a condition of project approval in order to mitigate or avoid significant effects on the �
environment."A Mitigation Monitoring and Reporting Program(MMRP)is required for the proposed project
� because the IS/MND for the project identified potentially significant adverse impacts related to construction and
-i implementation activities,and mitigation measures have been identified to mitigate those impacts.

i This MMRP has been prepared to ensure that all required mitigation measures are implemented and completed i
� according to schedule and maintained in a satisfactory manner during the construction of the project,as required. !
' The MMRP may be modified by the District during project implementation, as necessary,in response to changing
` conditions or other refinements.A table(attached)has been prepared to assist the responsible parties in �
implementing the MMRP.The table identifies individual mitigation measures,monitoring/mitigation timing,and
the person/agency responsible for monitoring. The numbering of mitigation measures follows the numbering ,
sequence found in the IS/MND.

� ROLES AND RESPONSIBILITIES
Unless otherwise specified herein,the construction contractor is responsible for taking all actions necessary to
implement the mitigation measures according to the specifications provided for each measure,and for
¢ demonstrating to the District that the action has been successfully completed.

� MITIGATION MONITORING PLAN TABLE
� The annual report submitted will verify the implementation of mitigation measures. The MMRP Table that
� follows will guide the District in their evaluation and should be the basis for annual reporting.
The column categories identified in the MMRP Table are described below.
Mitigation Measure-This column provides the text of the mitigation measures identified in the MND.
Timing-This column lists the time frame in which the mitigation will take place.
' Monitoring Agency-This column identifies the entity responsible for ensuring all requirements of the
mitigation measure are complied with. In most cases,the construction contractor will be responsible for
� implementing the mitigation measure.The District has indicated that it will take responsibility for monitoring the
� construction of the proposed project to ensure adherence to construction plans and specifications.

PROJECT: Upper Truckee Marsh Sewer Facilities Protection Project

PROJECT DESCRIPTION
This combined Initial Study and Mitigated Negative Declaration(IS/MND)evaluates the environmental effects of
the proposed Upper Truckee Marsh Sewer Facilities Protection Project.The project involves implementing an
adaprive management plan(AMP)to protect its existing force and gravity sewer mains along with its Bellewe
Puxnp Station facility in South Lake Tahce,California.The AMP consists of ineasures designed both to prevent
permanent establishment of Trout Creek over the sewer lines and adjacent to the pump station facility and to
encourage flows to establish new paths to the south,away from the District's faciliries.
The proposed project is located on property owned by the California Tahoe Conservancy at the north margin of
the Upper Truckee Marsh.The study area includes 96 acres along Trout Creek and is generally bounded by U.S.
Highway 50 on the south,the A1 Tahoe neighborhood on the northeast,Lake Tahoe to the north,and the Upper
Truckee Marsh to the south and west.
FINDINGS
An IS/MND has been prepared to assess the project's potential effects on the environment and the significance of
those effects.Based on the IS/MND,it has been determined that the proposed project would not have any
significant effects on the environment after implementation of mitigation measures.This conclusion is supported
by the following findings:
1. The proposed project would have no effects related to mineral resources,population and housing,or public
services.
2. The proposed project would have a less-than-significant impact on aesthetics,agriculture and foreshy
resources,biological resources,geology and soils,greenhouse gas emissions,hydrology and water quality,
land use and planning,recreation,and utilities and service systems.
3. Mitigation is required to reduce potentially significant impacts related to air quality,cultural resources,
hazards and hazardous materials,noise,and transportation/traffic.
The following mitigation measures would be implemented by the South Tahce Public Utility District(District)to
avoid or minimize environmental impacts. Implementation of these mitigation measures would reduce the
environmental impacts of the proposed project to a less-than-significant level.
► MiNgation Measure AQ-1: Reduce ConstrucNon-Related Emissions of Fugitive Dust
The District and their construction contractor will comply with EDCAQMD Rule 202,Visible Emissions;
Rule 205,Nuisance;Rule 223,Fugitive Dust�eneral Requirements;and Rule 223-1,Fugitive Dust—
Construction,Bulk Material Handling,Blasting,Other Earthmoving Activities,and Carryout and Trackout
Prevention. In addition,the contractor will implement the following fugitive dust control measures:

• Apply dust suppression measures in a sufficient quantity and frequency to maintain a stabilized surface
t and prevent visible dust emissions from exceeding 100 feet in length in any direction.Apply water to at
least 80 percent of the surface areas of all open storage piles on a daily basis when there is evidence of
� wind-driven fugitive dust.
� • Install control measures immediately adjacent to the paved surface to prevent track-out from exiting
vehicles.
� According to EDCAQMD,implementation of these control measures is sufficient to reduce construction- �
` related emissions to a less-than-significant level.With implementation of these measures,the proposed �
project's construction activities would not violate any air quality standard or contribute substantially to an �
existing or projected air quality violation.Therefore,implementing Mitigation Measure AQ-1 would reduce �

� this impact to a less than significant level. I

_ ► Mitigation Measure CiJI.-1:Avoid Potential Effects on Previously Undiscovered Resources ',
g If buried or previously unidentified resources are discovered during project activities,all work within a 30- '
� foot radius of the find will cease.The District will hire a professional archaeologist meeting the Secretary of
`? the Interior's Professional Standards for Archaeologists to assess the discovery and recommend what,if any,
further treatrnent or investigation is necessary for the find.Any necessary treatment/investigation will be
:� completed before project activiries continue in the vicinity of the find. If the find is related to tribal uses,the
Washce Tribe of Nevada and California will be contacted and invited to consult with the hired professional
; archaeologist or monitor any further necessary treatment or investigation if needed.
'; Implementing Mitigation Measure CUL-1 would reduce the impact of the proposed project on previously
�
undiscovered historical resources to a less than significant level.
'; ► Mitigallon Measure CUL-2:Avoid Potential Effects on Previously Undiscovered Burials
`
In accordance with the California Health and Safety Code,if human remains are uncovered during ground-
disturbing activiries,the District and its contractor(s)will immediately halt potentially damaging excavation
in the area of the burial and will notify the El Dorado County Coroner and a professional archaeologist to
determine the nature of the remains.The coroner is required to examine all discoveries of human remains
� within 48 hours of receiving notice of a discovery on private or State lands(Health and Safety Code Section
7050.5[b]).If the coroner determines that the remains are those of a Native American,he or she must contact
� the NAHC by phone within 24 hours of making that determination(Health and Safety Code Section 7050[cJ).
`A After the coroner's findings have been made,the archaeologist and the NAHC-designated Most Likely
' Descendant will determine the ultimate treatment and disposition of the remains and take appropriate steps to
'x ensure that additional human interments aze not disturbed. The responsibilities of El Dorado County for acting
upon notification of a discovery of Native American human remains are identified in Section 5097.9 of the
:p California Public Resources Code.
� California law recognizes the need to protect Native American human burials,skeletal remains,and items
k associated with Native American burials from vandalism and inadvertent destrucrion.The District will ensure
that the procedures for the treatment of Native American human remains contained in California Health and
Safety Code Sections 7050.5 and 7052,and California Public Resources Code Section 5097,are followed.
� Implementing Mitigation Measure CUL-2 would reduce the impact associated with the project's potential to
� disturb human remains to a less than significant level.

_ The District's contractor shall be responsible for providing an approved traffic control plan subject to review
� and comment by TRPA and the CSLT before construction.The plan will address project construction traffic

and parking,and emergency access.At a minimum,the traffic control plan will discuss truck haul routes,truck
turning movements at the proj ect staging area,traffic control signage,potential bicycle and pedestrian traffic
conflicts,and monitoring of the in-place traffic control plan to implement traffic control revisions,if necessary.
Implementing Mitigation Measure HAZ-1 would reduce construction-related emergency response and
evacuation impacts to a less than significant level.
. Mitigation Measure NOI-1: Reduce Noise Levels from On-site ConstrucNon Equipment
The following noise-reducing construction practice will be implemented to reduce impacts on noise-sensitive
receivers during construction of the project:
• Before construction,all residences within 650 feet of construction areas will be notified in writing of the
proposed construction activities.Construction scheduling and contact information will be clearly
displayed on pedestrian signage.
Also,implementation of the following mitigation measures normally considered during construction activities
is recommended to reduce construction noise exposure:
• Plan noisier operations during times of highest ambient noise levels.
• Keep noise levels relatively uniform; avoid excessive and impulse noises. Operate equipment to minimize
banging,clattering,buzzing,and other annoying types of noises,especially neaz residential and other

• To the extent feasible,configure the construction site in a manner that keeps noisier equipment and
acrivities as far as possible from noise-sensitive locations and nearby buildings.
• Use construction equipment manufactured or modified to reduce noise and vibration emissions,such as
electric instead of diesel-powered equipment.

The District's contractor shall be responsible for providing an approved traffic control plan subject to review
and comment by TRPA and the CSLT before construction.The plan will address project construction traffic

truck turning movements at the project staging area,traffic control signage,potential bicycle and pedestrian

REQUESTED BOARD ACTION: Accept the proposal from the recommended consultant
to provide engineering services for the Sunset Well Evaluation Project �Project).
DISCUSSION: The Sunset Well was constructed in 1990 and is currently used as a lag well
to the Main Zone in the District's water distribution system. As part of the District's
update to the water distribution system master plan, improved water production from
this site is being considered as an alternative to increase available water supply for use
in the South Y area.
Well information from this site is dated and limited to production tests performed
following initial well installation in 1990. In order to collect the information needed to
assess potential for improved water production, District staff prepared a Request for
Proposal (RFP) seeking a qualified engineering firm that could complete a detailed
evaluation on the current condition of the well; restore the well to reestablish well
efficiency and flow conditions; and evaluate the restored well to ascertain potential
yield and need for a possible replacement well.
On March 27, 2014, the Engineering Department posted the RFP on the District's
web site and notified seven (7) engineering firms with experience in well evaluation
and restoration projects that could potentially assist staff to complete the Project.
The RFP requires interested parties to submit a separate Technical Proposal and an
accompanying Cost Proposal for staff review and evaluation by no later than
April 23, 2014.
Consultant selection will be qualification-based, solely upon the information provided in
the Technical Proposal. A summary of the proposals and staff's recommendation will be
provided prior to, and at the Board Meeting.
Funding for this project will be provided on a 50% cost share basis with the EI Dorado
County Water Agency (EDCWA).

2 BACKGRUUND

2.1 Sunset Well
� The project is located within a residential neighborhood near the intersection of Sunset
,r
Drive and Conestoga Street,within Section 4,T 12 N, R 18 E, MDBM in the City of South
; Lake Tahoe, EI Dorado County, CA(Figure 1).The Sunset Well is situated on a 40' x 80'
permanent easement in the south corner of a 10-acre parcel (APN 031-29-011). Access
;� to the well is via U.S. Highway 50 to Silver pollar Avenue to Sunset Drive (Figure 2). The
wellhead is located outside the adjoining well house and is enclosed using a pitless
� adapter(Figure 3).The well discharge line consists of 6-inch epoxy coated welded steel
�
f pipe,with an automated flow control valve,flow meter,chlorination and pressure
' appurtenances situated in a concrete vault underlying the well house.
�;
`t The Sunset Well was constructed in 1990 in a 22-inch diameter borehole drilled to a
total depth of 600 feet(Figure 4).The nominal 24-inch conductor casing extends from
� ground surface to a depth of 50 feet below ground surface (ft bgs).The production well
(10.75-inch O.D.) is constructed using copper-bearing steel blank casing(0.250-inch
wall) and Type 304 stainless wire-wrapped well screen (0.040-inch).The screen interval
extends from 275 to 430 ft bgs.A 1.5-inch sounding tube is present at ground surface
� next to the pitless adapter.The sounding tube extends to a depth of more than 256 ft
bgs. It is not known if the sounding tube penetrates into the production casing.
; The annular space surrounding the well casing is cemented from ground surface to a
depth of 255 ft bgs.The filter pack(8 x 20 Lone Star) surrounding the screen interval

extends from 255 ft bgs to 440 ft bgs.The borehole is sealed below the bottom of the
well with a cement-bentonite grout from 440 ft bgs to total depth.
The well is equipped with a 100 hp Gould Model VIS submersible well pump and motor
; coupled to a nominal 6-inch column pipe.The well pump is rated at a discharge capacity
� of 750 gallons per minute (gpm) at a total head of 340 feet.The intake is set at a depth
of approximately 250 to 260 ft bgs. Pressure on the water system side of the discharge
:� line is typically about 84 psi.
Water from well production testing will need to meet California drinking water
� standards prior to discharge to the District's water distribution system.

A hydrograph showing static water levels and monthly production for the Sunset Well is
provided below(Figure S). Most recent measurements(on March 24, 2014) show well
a static water level at 21.05 ft bgs.
The Sunset Well is used as a lag well in the District's water system.Since 1996,daily
production from this well has averaged about 0.69 million gallons per day(MGD).
Monthly production from this well ranges from 0.02 MG to 29 MG,with a median value
; of about 12.88 MG.Average daily demands typically increase during the summer season
from May through September(Figure 6).

Aquifer test information for the Sunset Well is limited to two data sets collected in 1990
during initial testing of the production well. An approximately 16-hour constant rate
test appears to have been performed in July 1990. A second test was performed in
September 1990, which consisted of a 9-hour step test and a 2-hour constant rate test.
Aquifer transmissivity derived from the recovery data and specific capacities derived
from the pumping data are summarized below.

There is no other aquifer or weil test data found for this well. Since 1990,there have
been no down-hole camera inspection surveys,well redevelopment or well
rehabilitation work performed on this well.
The Sunset Well has a hydrogen sulfide odor problem. Sodium hypochlorite used for
disinfection is also used to chlorinate the hydrogen sulfide,thereby removing the odor
prior to discharge to the water distribution system. General water quality information
for the Sunset Well is summarized in the following table (Table 1). Organic contaminants
have not been detected in this well.

2.2 Waste Discharge
All waste fluids from the Sunset Well evaluation project will need to be properly
handled, contained and neutralized (if needed) on-site, prior to discharge to the
District's sanitary sewer collection system. Discharges to the collection system will need
to be sediment free having a turbidity of less than 300 NTU. The sewer collection system
may accommodate flows of less than 200 gallons per minute (gpm) from the project
location.
3 SCOPE OF WORK
The scope of work will generally consist of the preparation of work plans and technical
specifications; and technical oversight during well rehabilitation and well profile
evaluation for the Sunset Well Evaluation Project(SUNSET). An abbreviated scope of
work for this project is provided below.The consultant is encouraged to expand on this
scope, as it deems necessary,to provide a complete work product. As currently
envisioned,the project will entail the following tasks:
1. Data Review: Conduct a site visit and review available physical and chemical well
data (water well drillers report, electric logs, water quality results, pumping test
results)to gather sufficient information for development of a work plan for well
rehabilitation and profile evaluation;
2. Work Plan: Complete a work plan for aquifer testing,well rehabilitation and vertical
flow and water quality profile evaluation.The work plan shall include:
2.1. Proposed methods and equipment for well inspection;
2.2. Proposed methods and equipment for aquifer testing
2.3. Proposed methods and equipment for mechanical development;
2.4. Decision tree showing proposed equipment and methods for chemical
development;
2.5. Proposed methods and equipment for vertical flow and water quality profiling;
2.6. Proposed methods and equipment for pump removal/re-installation;
2.7. Proposed methods and equipment for well disinfection.
2.8. Technical specifications to be used by sub-contractors employed by the
consultant to complete aquifer testing,well inspection,well restoration,vertical

well profiling, and well disinfection.The District will be responsible for providing
laboratory analytical services for this project;
2.9. Proposed methods and equipment for waste management and disposal;
2.10. Proposed methods and calculations used for vertical profiling data
evaluation;
2.11. Proposed schedule showing start and finish dates for each major task;
2.12. Itemized budget, including all labor, material,tools and equipment,
required to complete the work.The budget estimate shall be provided on a time
and materials, not to exceed cost basis.
3. Well Evaluation: Complete the aquifer testing,well inspection, well restoration and
well profile evaluation in accordance with the approved work plan using California
licensed sub-contractors retained by the consultant;and
4. Technical Report Prepare a technical report describing:the aquifer testing,well
;3 inspection,well restoration and well profile evaluation work; summary of the well
evaluation findings;and potential for additional water supply at the project location.
All supporting documentation, including video well inspection reports,field records,
� photos, flow logs and laboratory reports shall be presented in appendices to the
��' technical report.

4 DELIVERABLES
At a minimum,the Consultant shall be required to provide the following deliverables for
the project:
1. Well Evaluation and Restoration Work Plan including data collection and
evaluation methods, specifications, proposed schedule and budget estimate; and
2. Well Evaluation and Restoration Report including permits,field notes, work
chronology,findings, calculations and supporting documentation.
5 PROJECT TEAM
Funding for this project is being provided on a 50�o cost share basis by the EI Dorado
County Water Agency(EDCWA), using an anticipated cost of$120,000.This represents
the District's preliminary estimate of the level of effort that may be needed for this
project.

Mr. Paul Sciuto, P.E., Assistant General Manager,will be the District's Project Director.
Mr. Sciuto will have overall responsibility and accountability for the project. Mr. Ivo
Bergsohn, C.Hg. Hydro-Geologist, will be the District's Project Manager and will be the
principal point of contact for the selected consultant. Mr. Bergsohn will be responsible
for directing the project through its various phases, including, coordinating technical
efforts to insure that the project is completed in a timely and cost effective manner.

Experience and familiarity with the public agencies working in the greater South Lake
Tahoe Area; including:the Lahontan Regional Water Quality Control Board (LRWQCB)
and City of South Lake Tahoe (CSLT) are also desired.
6 PROPOSAL
Consultant selection will be qualification-based, along with the proven ability and
experience to provide the services required for this project. Therefore,the District is
requesting that interested firms submit separate Technical and Cost Proposals for this
project. Further explanation for each proposal type is provided below.
6.1 Technical Proposal
The Technical Proposal shall be limited in format and length. Format will be 8-1/2"x 11"
with foldout sheets allowed up to 11"x 17" in size.All foldout sheets up to a maximum
of 11" x 17" sheets will be counted as two pages and shall be labeled as such. Length of
the proposal shall be limited to a maximum of eight(8) numbered pages of text no
smaller than 11 point, and/or graphics. Material excluded from the eight(8) pages
maximum count is limited to:
• Front cover
• Divider pages
• Back cover

6.1.1 Format

The Technical Proposal shall conform to the following format:
• Submittal Letter: Each proposal must be accompanied by a submittal letter.The
submittal letter shall identify the name and title of the person authorized to
contractually obligate the firm for the purpose of this RFP and the person(s)to be
contacted for clarification questions regarding the proposal.The submittal letter
shall also include any exceptions taken to the District's Standard Consulting
Agreement (Attachment A).
• Firm Experience(no more than 1 page): Include a general overview of your firm
and the firm's experience by providing examples of highly successful aspects of

projects similar to this project completed by the office(s) submitting the proposal.
Describe specific relevant experience of project team members and how that
'' expertise will be utilized in this project. List at least three (3) relevant public water

supply well evaluation projects successfully completed by project team members
over the past five (5)years.
• Firm's Approach (no more than 4 pages): Include a description of:the key project
elements/goals and how the proposed approach will meet the District's
expectations in a timely and cost efficient manner. Provide a description of the
equipment and methods proposed for well inspection,well testing, restoration,well
evaluation and handling and disposal of waste discharges. Manufacturer/supplier
information describing all major items of equipment proposed to be used for well
inspection, restoration,well evaluation and waste management shall be provided as
attachments in the Appendix. Identify challenges that might be expected and

possible solutions to resolving those challenges within the time allotted in the
project schedule.
• Project Team (no more than 2 pages): Describe your project team, including title,
office location and contact information. Include the key individuals and sub-
contractors that will actually be responsible for conducting the project's technical
activities and management of the project. An organization chart should be provided
showing the inter-relation of all the project team members, both District and
Consultant.
• Work Estimate Table: Using the Firm's Approach, provide an initial estimate
showing the level of effort(in work-hours) needed to complete the project.The
estimate should be presented on a line item, per task basis, showing the number of
work-hours required for each project team member and sub-contractor(s);the total
work-hours required for each task;the total work-hours required for the total
project. Unit labor, expenses and subcontractor costs are not to be included in the
Work Estimate Table.
• Project Schedule: Provide a project schedule listing major tasks,deadlines, duration
and projected start and finish dates for the project.
• Appendix: Resumes of proposed key staff relevant to the scope of work; manufacturers
information describing all major items of consultant and subcontractor(s)equipment
proposed for work on the project;and an example Well Evaluation and Restoration report,
prepared by the project team members.
6.2 Cost Proposal
The Cost Proposal shall include the following items for each of the major tasks and
activities described in the accompanying Technical Proposal:
• Personnel, estimated labor hours and hourly rates for each member of the
project team;
• Project expenses;
• Sub-Contractor costs and
• Total costs.
Cost Proposals will not be opened until after each firm's Technical Proposal has been
carefully reviewed and the firm deemed most qualified has been selected. Only the
Cost Proposal for the selected firm will be opened and will form the basis for negotiating
the contract. Unopened proposals will be returned.

7 EVALUATION
All proposals will be evaluated using the foilowing criteria:
• 10 Points: Proximity to or familiarity with the area and regulatory agencies in
which the project is located.
• 15 Points: Capacity and capability of the firm, including sub-contractors,to
perform the work, including any specialized services,within the given time
limitations.
• 20 Points: Project needs and the specialized competence(s) of the firm.
• 25 Points: Proven experience in municipal well evaluation and restoration.

• 30 Points: Firm's approach including evidence of understanding of scope of work,
project needs, limitations and requirements.
8 SCHEDULE
EDCWA Grant Funding constraints require that work for this project be completed by
June 30th,2014. In order to complete the project within the allowed time frame,the
consultant will be expected to complete project deliverables in a timely manner in
general accordance with the following project milestones:
• Kick-Off Meeting 05/5/2014
• Work Plan Review Meeting 05/23/2014
• Well Restoration & Evaluation 05/28/2014-6/30/2014
• Project Report Submittal—final 7/31/2014

DISCUSSION: The negotiating teams met on April 22, 2014, and the Union provided the
District negotiating team with a proposal to be discussed with the Board in Closed
Session. A cost estimate is also being provided to the Board by the District negotiating
team, as well as information provided by Koff 8� Associates regarding industry market
data and COLA's.
