SUBJECT: SUMMARY OF 13267 SITE HISTORY QUESTIONNAIRES AS OF
JULY 26, 2019
The purpose of this memorandum is to summarize the status of the Water Code section 13267
Investigative Orders (Orders) sent on April 3, 2019 requiring potential responsible parties
identified to complete a General Chemical Storage and Use Questionnaire or a Dry Cleaner
Specific Questionnaire (Site History Questionnaire) by May 3, 2019. The intent of requiring the
submittal of the Site History Questionnaires was to identify other potential sources that may be
contributing to the regional tetrachloroethylene (PCE) contamination that is impacting or
threatens to impact the municipal, private, and small community water supply wells in the South
Y area. Information obtained from the completed Site History Questionnaires received is
supporting the regional PCE contamination characterization efforts currently being conducted
using funding from the 2019 grant issued by State Water Resources Control Board’s Site
Cleanup Subaccount Program.
Orders were sent to potential responsible parties identified by completing a records search
(Tier 1 Evaluation) for businesses that may have used, stored, handled, or disposed of
chlorinated solvents within the regional PCE contamination area in the South Y area. The Tier 1
Evaluation identified over 200 businesses in the South Y area that may have used chlorinated
solvents and these included historic and current dry cleaners, laundromats, carpet cleaning
businesses, automotive repair shops, paint and auto body shops, and printing shops. The
Orders issued, Site History Questionnaires received, and Notice of Violations (NOVs) issued as
of July 26, 2019 are summarized in Table 1 (Attachment 1). The Site History Questionnaires in
Attachment 1 include supporting documents provided (e.g. Safe Data Sheets, Reports,
Hazardous Materials Business Plans, Phase I Assessments, Site Assessment Reports, etc.)
received as of July 26, 2019. Below is a summary of the items completed to date since issuing
the Orders, and proposed future action items.
• 223 Orders were issued on April 3, 2019.

• 74 Orders were returned as undeliverable due to invalid contact information for the
potential responsible party. The Order mailing list was regenerated by reviewing historic
records (e.g. Lahontan Water Board files, environmental database searches) and
identifying fee title owner of current or former business address using LandVision.
Following the review of the 74 Orders returned as undeliverable, 54 of the issued Order
were rescinded because Lahontan Water Board staff determined the Order was satisfied
or can be satisfied under a separate Order. Contact information for the remaining 20
undelivered Orders, may be researched in the future if Lahontan Water Board staff
determines it to be necessary.
• 99 Site History Questionnaires were submitted to Lahontan Water Board staff by
June 12, 2019. Information received is currently being evaluated to identify
businesses/properties that may have used, stored, or disposed of chlorinated solvents.
• 29 Notices of Violation (NOVs) were issued on June 12, 2019 to potential responsible
parties who did not complete and submit the required Site History Questionnaire.
• 16 Site History Questionnaires were submitted to Lahontan Water Board staff following
the issuance of the NOVs. Information received is currently being evaluated to identify
businesses/properties that may have used, stored, or disposed of chlorinated solvents.
• Lahontan Water Board staff intends to issue additional NOVs to 11 potential responsible
parties who did not comply with the April 3, 2019 Order or the June 12, 2019 NOV.
• Site History Questionnaires received are under review and the information included in
them is being evaluated. Properties suspected to be contributing to the regional PCE
contamination, may be required to conduct a Site Assessment to determine if a release
has occurred from the subject property.
